NDIS Referral Form
You've got a participant who needs physiotherapy, and what you need to know is whether someone will pick the referral up and actually get them seen. Send it through here and we'll come back to you with what we can realistically commit to.
Who this form is for
Use this form if you're a support coordinator referring a participant, a plan manager sending someone our way, or a participant or family member arranging physiotherapy from a self-managed or plan-managed budget.
Two quick exits, so you're not filling in the wrong thing:
- Referring someone whose care isn't funded through the NDIS (a GP, specialist or allied health referral), use Make a referral instead.
- Already a patient here and you just need a time? Book an appointment is faster.
Well Motion Physiotherapy & Allied Health is a registered NDIS provider. How the funding side works, and what a program with us actually looks like, is set out on our NDIS Physiotherapy page.
What to have ready
None of this has to be perfect. Send what you've got and we'll work out the rest with you.
- The participant's details: name, contact, and the best person to reach if it isn't them.
- Their plan goals, or the wording from the goals section. That's what the program gets built toward, so it matters more to us than a diagnosis does.
- The funding arrangement: plan-managed, self-managed or NDIA-managed, and who we invoice. If they're plan-managed, the plan manager's details save a round of emails.
- Your own contact details if you're coordinating, so we can keep you in the loop on progress and reporting without anyone relaying messages.
- Anything already written, if it exists: previous reports, scans, a specialist's letter, an old exercise program that didn't stick. Optional, and genuinely useful: it saves the participant retelling their whole story. A GP referral isn't needed to see a physiotherapist.
Send the referral
Stuck on anything? Call (02) 8111 5633 and say it's an NDIS referral.
What happens after you send it
We'll confirm availability and tell you what we can commit to. A same-week first appointment is usually possible across our four clinics, and if travel is the barrier, we can see the participant at home through mobile physiotherapy or by telehealth.
From there we report against the goals in the plan rather than against session counts, in language a plan reviewer can use.
If we're not the right fit (wrong location, wrong clinical need, wrong timeframe), we'll tell you quickly rather than accept a referral we can't service properly. You'd rather know this week than in a month.
If you'd rather talk it through first
Call (02) 8111 5633, one number for all four clinics, and say it's an NDIS referral. Hours differ by clinic and are listed on our clinic locations page. For anything that isn't a referral, our contact page has the rest.
